How do I create a simple HTML form with e-~

Hi, I want to attach a web page to an e-mail, for a survey and would like
readers to select some multiple choice answers and then click the submit
button. It should then email the answers back to me.
Is this possible in HTML?
I am a novice and would like the form to work with Netscape as well as IE.

If you put in an absolut reference to a serverside formmail script, that
powers your form, and your respondee is online when she click the submit
button it should be possible. If you can find a server/ISP wich support Matt
Wrights Formmail http://www.worldwidemart.com/scripts/, it should be
compatible with all browsers. But you have to get some help on this, if
your'e a novice. Good luck.
